Primary Care Networks are groups of family doctors who work alongside nurses, behavioural health consultants, pharmacists, social workers and other health professionals to meet the everyday health needs of patients in the communities in which they live and work.

PCNs develop specialized programs and services that help improve access to care, cut wait times for specialists and reduce healthcare costs. In Central Zone, that includes free workshops, clinics focused on specific needs, innovative programs, improved continuity and transitions in care, and partnerships with the healthcare system, community resources, and local hospitals.

Research shows that patients who have a continuous relationship with a family doctor and team are healthier as they age and live longer. They receive better care, make fewer visits to emergency rooms and are hospitalized less. Your family doctor and your PCN are working together to give you and your family a home base for a healthier future.

About Central Zone PCNs

There are 12 PCNs in the Central Zone, stretching across the province between Edmonton and Calgary. They work alongside over 350 physicians to provide primary care for nearly 400,000 patients. Central Zone PCNs works with rural communities to develop programs that are designed for those communities specifically and address local concerns. As well, the PCNs in Central Zone work together to bring solutions to widespread issues. Central Zone is one of five in Alberta. There are 39 PCNs province-wide.
